Registration form for the 2010 / 2011 season

1. Registration will be held at the time of tryouts in September
2. Before the girls step on the ice tryout fees have to be paid in full $50 this will entitle you to get a  minimum 3 hours of tryouts
3. A registration form MUST be signed by a parent before stepping on the ice for the tryouts  including ALL midget players,  intermediates if they are over the age 0f 18 can sign their own form.  Please print off the form and have it ready for registration – this way the older players who come on their own can have it already signed.
4. A ‘Parental Code of Conduct’ must be signed by all parents once the team rosters have been finalized
5. Registration fees are 500.00 payable in cash or cheque (3 postdated cheques will be accepted September 15, October 15, November 15)
6. Rep fees will need to be paid in full once the rosters have been finalized, Rep fees for Novice - Bantam are $75.00 and Midget A is $125.00
